Adoption Application
Thank you wanting to provide a forever home for one of our beloved furry friends.
Please feel out the application below and read the terms and conditions. Printing your name on the last line will indicate you agree to all the terms and conditions for adoption. We will review you application and check the references provided as quickly as we can so as not to delay your time away from your new best friend. We will contact you as soon as the application has been received and again once it has been approved.
Terms and Conditions
Ownership Transfer - The adopter accepts full legal responsibility and ownership of the listed animal as of the date of the finalized adoption agreement.
Health Disclosure - The rescue affirms that, to the best of its knowledge, the dog has received necessary veterinary assessment. The adopter accepts the dog in its current condition and understands that future heath issues may arise.
Veterinary Care - The adopter agrees to provide appropriate ongoing veterinary care, including parasite prevention and wellness check-up
Spay/Neuter Agreement - If not currently spayed/neutered, the adopter agrees to have this procedure done by the the estimated 1 yr birthdate
Restrictions on Transfer - The adopter agrees not to abandon, sell, gift, or otherwise transfer ownership of the dog without prior written permission from the rescue. If the the adopter cannot retain the dog, it must be returned to the rescue.
Training and Socialization - The adopter acknowledges the dog may require behavioral training and agrees to use only positive, humane training methods.
Adoption Return Policy - The dog may be returned to the rescue at any time if necessary. NO REFUNDS will be given under any circumstances of any fee paid by
adopter.
Living Conditions - The dog shall be kept as an indoor companion pet (unless approved otherwise) and provided with clean shelter, nutritious food, fresh water, and humane treatment at all times.
Home Visit Requirements - The adopter agrees to allow a pre-adoption home visit (in-person or virtual) to ensure the environment is safe and suitable. A post-adoption follow-up visit may also be conducted within 2–4 weeks after placement to assess the dog’s adjustment and well-being. The adopter agrees to cooperate with scheduling and completion of these visits.
Liability Waiver - The adopter releases the rescue organization from all liability related to the dog’s behavior, health, or actions after adoption.
Reference Check - The adopter agrees to provide at least two personal references who can attest to their ability to provide a stable, loving, and responsible home for the dog. The rescue organization reserves the right to contact these references prior to finalizing the adoption.
